Evaluation Surveys
After you have watched the entire training video, there is a link at the bottom of the page for an evaluation survey. We very much apprecaite your feedback, so please take the survey, and make sure to enter "United Church of Chapel Hill" in the organization affiliation box. Your responses are anonymous; we are seeking only to track how many people from the United Church of Chapel Hill are participating in this program and to track survey responses as a whole from your community. Certificates of completion will be created for each individual who completes the training and takes the survey (while maintaining your confidentiality), and will be forwarded to the United Church at Chapel Hill.
